Richland County restaurant inspections: critical violations April 21-27

The following restaurant inspections with critical violations were conducted by Richland Public Health between April 21 and April 27:

• North Central State College/Ohio State University-Mansfield Child Development Center, 2441 Kenwood Circle, Mansfield, April 21. Food employee(s) did not wash hands when required (critical, corrected during inspection). Employee touched her mask and started to touch clean equipment without washing hands. 

• Logan's Roadhouse Inc., 2170 W. Fourth St., Ontario, April 22. Person in charge (PIC) unable to demonstrate knowledge of cleaning and sanitizing (critical, corrected during inspection). PIC unable to demonstrate knowledge of correct strengths for lactic acid sanitizer used. EH Specialist provided correct test strips and information on what is needed for testing for strength. Handwashing sink is being used for purposes other than handwashing (critical, corrected during inspection). Hand sink dish area was observed to have ice and other food debris. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Hamburger patties observed in walk-in meat cooler uncovered. 

• Scarlet and Gray Café, 1760 University Drive, Mansfield, April 22. Improper storage of poisonous or toxic materials (critical repeat, corrected during inspection). Observed working sanitizer buckets to be stored on shelf with single use items. Presence of live insects, rodents and other pests (critical repeat). Observed multiple droppings on shelving below register counter where single use items are stored. Clean surfaces and verify services of licensed pest control by April 26.

• Springmill Corner Market, 939 Springmill St., Mansfield, April 22. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Smoked pork neck bones with a cooking temperature of 160 degrees observed above whole cuts of meat in retail reach-in cooler. No air gap or approved backflow prevention device on the plumbing system (critical). Mop sink was not equipped with back flow. Correct by April 29.

• Mickey Mart Bellville No. 21, 750 Ohio 97 W., Bellville, April 22. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Rack in the pizza oven has production build-up. 

• Mickey Mart No. 32, 61 Mill Road, Bellville, April 22. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Rack for the pizza oven had visible pizza build-up. 

• McDonald's - Appleseed No. 13982, 1480 Lexington Ave., Mansfield, April 25. Chlorine sanitizing solution at incorrect temperature and/or concentration (critical, corrected during inspection). Chlorine solution in wiping cloth bucket was at less than 10 ppm. PIC had remade to manufacturers strength of 100 ppm. TCS foods not being hot held at the proper temperature (critical, corrected during inspection). Sausage gravy in hot holding unit was 110-123 degrees. PIC voluntarily discarded 5 pounds of gravy as product was below 135 degrees for an undetermined amount of time. Improper use of time as a public health control, 4 hours (critical, corrected during inspection). Cheese held on prep line was not marked with the time it was placed out. 

• Wilging's Fine Meats, 1391 Lexington Ave., Mansfield, April 25. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Raw eggs observed above pickles in walk-in cooler. PIC moved so that raw eggs were not above ready-to-eat foods. 

• Top Hat, 1901 Ashland Road, Mansfield, April 26. Direct connection between the sewage system and a drain originating from equipment in which food, portable equipment or utensils are placed (critical). Observed improper air gap on ice machine outflow pipe. 

• Whit's Frozen Custard, 1531 Lexington Ave., Mansfield, April 26. PIC unable to demonstrate knowledge of cleaning and sanitizing (critical, corrected during inspection). PIC unable to state the correct strengths that sanitizer is made up in facility. PIC unable to demonstrate knowledge of temperatures and times for safe refrigerated food storage, hot holding, cooling and reheating (critical, corrected during inspection). PIC unable to state the temperatures and times for the parameters to properly cool foods. Cooling log and information provided at time of inspection. Food not properly protected from contamination by separation, packaging and segregation (critical repeat, corrected during inspection). Several items in walk-in freezer observed to be uncovered. 

• Madison South Elementary, 700 S. Illinois Ave., Mansfield, April 26. Quaternary ammonium sanitizing solution at incorrect temperature, concentration, and/or water hardness (critical, corrected during inspection). Sanitizer concentration in three compartment sink measured less than 100 ppm. Sanitizer added to register over 200 ppm. 

• Subway Sandwich Shoppe No. 28057, 1135 Park Avenue West, Mansfield, April 26. Food not properly protected from contamination by separation, packaging and segregation (critical, corrected during inspection). Observed a tray of uncovered cookies on the counter. PIC placed cookies in the covered display case. Equipment food-contact surfaces or utensils are unclean (critical, corrected during inspection). Observed a bread pan with food crumbs on the clean pans rack. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Observed a spray bottle of oven cleaner hanging on a shelf above clean pans. PIC moved bottle of cleaner to the chemical storage area. Improper storage of poisonous or toxic materials (critical, corrected during inspection). Observed a container of coffee pot cleaner stored on a shelf above the food prep sink. Food contact surfaces not easily cleanable (critical). Observed a cracked plastic container by the ice machine. Correct by May 3.

• Ontario Lions Club, 3708 Snodgrass Road, Ontario, April 26. Evidence of rodents and other pests (critical, corrected during inspection). Observed droppings in lower cabinets. PIC properly cleaned.
